JAMES DOYLE CASE MEMORIAL SCHOLARSHIP
INSTRUCTIONS

Complete the Scholarship Application. (Must be typewritten. Handwritten applications will not be accepted.)

Provide a brief Autobiographical Sketch: No more than two (2), double-spaced, typewritten pages containing information about you. Tell us about your personal goals, strengths, weaknesses, hobbies, honors, achievements, etc. Be sure to list the field or course of study you are pursuing and explain why you have chosen it.

Entering college freshmen should include a certified high school transcript. College sophomores, junior or seniors should provide certified transcripts from the school currently attending or have most recently attended. Graduate students should include undergraduate transcripts as well as an up-to-date transcript of any graduate work already completed.

Copies of ACT or SAT scores for Undergraduate Students. Copies of GRE, MCAT, LSAT scores, etc., for Graduate Students.

Two letters of recommendation.

Proof of acceptance from a post-secondary school. Entering and transferring students must submit a notice or letter from the admissions office certifying acceptance or consideration for admission.

Statement from a qualified physician verifying the presence of legal blindness for the qualifying person (i.e., applicant or parent or legal guardian).

Completed application and required materials must be postmarked no later than March 1 each year and mailed to the following address:
